Urinary incontinence (UI) is the involuntary leakage of urine, affecting quality of life, especially in older women and men with prostate issues.
Types
Causes and Risk Factors
Common causes include weakened muscles, neurological disorders (e.g., stroke, MS), pregnancy, obesity, infections, medications and aging.
Assessment
QUID stands for the Questionnaire for Urinary Incontinence Diagnosis (also called the Questionnaire for female Urinary Incontinence Diagnosis). It is a short, validated 6-item self-report tool used primarily to help distinguish between stress urinary incontinence (SUI) and urge urinary incontinence (UUI) in women, and to assess symptom severity.
Management
Start with lifestyle (weight loss, fluid management), pelvic floor exercises; escalate to meds, surgery for severe cases.
We, at Sun Hospital Patna, India, carried out a Randomised Control Trial (RCT) taking 150 women suffering from urinary incontinence between the ages of 30-60 years. They were divided into groups of 75 each. The first 75 women comprised the control group. Other 75 were divided into two groups of 30-45 years & 46-60 years.
The types of Incontinence were studied among the two groups which concluded that stress urinary incontinence was more prevalent in younger age groups as compared to the older age group in which Urge was more predominant & most women from both the groups suffered from Mixed Incontinence.
Three modalities – Diode Laser therapy (DLT), Platelet Rich Plasma therapy (PRP) & Pelvic Floor Chair (HIFEM therapy) were applied on these women to look for improvement in urinary incontinence.
Diode Laser therapy with a wavelength of 1470nm at 17W, 5Hz & 50ms pulses was applied monthly for 3 months. Platelet rich plasma therapy was used paraurethrally monthly for 3 months & HIFEM therapy twice weekly for 6 such sessions. All subjects were followed at 3 months, 6 months & 1 year.
Conclusion
We concluded that Stress urinary incontinence predominantly improved with diode laser therapy & PRP therapy where as urge incontinence improved significantly with Diode laser therapy & Pelvic floor chair. Mixed incontinence showed improvement with all the three modalities.
